Please notify the RCS office of any phone number/contact changes. It is essential that home and work numbers for parents and emergency contacts are correct.
IMMUNIZATION REQUIREMENTS
Students entering preschool and grades PreK-12 must be fully immunized according to NYS Public Heath Law. Please check with your healthcare provider to ensure your child meets age/grade requirements
Students not in compliance may NOT attend school until the requirement is met.
MEDICATION IN SCHOOL
The NYS Education Department requires new medication orders every academic year. A provider’s order and parent permission are required for all prescription and non-prescription medication if taken during the school day.
MEDICATION IN SCHOOL POLICY

Written authorization from the prescribing physician which includes:

Student name
-
Birth date
-
Medication name
-
Medication route
-
Time of day to be administered or instructions for as medications that are “as needed”

Provider’s signature, title, address and phone number.

Written authorization from a parent/guardian.

Prescription medication must be in the original pharmacy container, labeled with the student’s name, medication, and dose. Non-prescription medication must also be in the original container with the student’s name on the bottle/box.

All medication must be delivered to the health office by the parent/guardian. Students are NOT permitted to carry any medications to school.

All medications will be stored in the health office in a locked cabinet.
STATE SCREENING MANDATES
NYS mandates Vision, Hearing and Scoliosis screenings – Parents/Guardians will be notified of any significant findings
Required Physical and Dental exams for grades: K-1-3-5-7-9-11
